How do you write a formal invite?

My son Kyles bar mitzvah is comming up and im not totally sure on what to include on the invite. I know date, time, place, ect. But how do i write this formally? Is thier anything else i should include with it?

Reply:First of all, it's called an invitation! Never an invite. That word is always a verb.
